This week I want to talk about something I’m seeing more and more in my clinic: repeated failed implantation.
It’s becoming such a familiar story. Everything looks perfect. The preconception work is done. The lining is great, the embryo is good, yet implantation keeps failing. And if that’s you, I want to share my honest opinion:
If you’ve had at least one miscarriage, or a failed implantation with a genetically normal embryo, there’s a good chance this is autoimmune infertility.
And in my experience, the standard protocols with progesterone alone aren’t enough. We’re living in a time of widespread immune dysregulation, and I truly believe this is the missing link for so many women.
What I Tell My Clients to Ask
When I’m guiding women through this, here are the steps I usually recommend:
- Ask for natural killer (NK) cell testing. This has to be done via an endometrial biopsy, not a blood test. When NK cells are high, your immune system basically sees a healthy embryo as “foreign” and blocks implantation.
- Ask if your clinic uses an autoimmune protocol for implantation support.
- If they don’t, consider getting a second opinion. Every doctor practices differently, and if you’re stuck getting the same result over and over, it’s time to explore other options.
Treatments I’ve Seen Make a Difference
Not every clinic offers these, and you’d never use them all at once. But these are the treatments I most often see helping my clients, and increasingly the very thing that makes the difference:
- Intralipid Infusions: calm overactive NK cells, often given before transfer and sometimes again in early pregnancy.
- Plaquenil (Hydroxychloroquine): not commonly used, but in the more complex cases I’ve worked with, it seems to do wonders.
- Steroids (Prednisone / Prednisolone): this is the most common, used in many clinics to lower inflammation and quieten immune activity.
- Clexane: a low-dose blood thinner that helps blood flow and prevents tiny clots that could stop implantation.
- IVIg (Intravenous Immunoglobulin): a more intensive option, only offered by a handful of doctors in Australia. If you’re curious, reply to this email and I can point you in the right direction.
- LMIT (Lymphocyte Membrane Immunotherapy): still new here, but available with some specialists. Designed to help your immune system “accept” the embryo. Again, let me know if you want more info.
Here’s the thing: these aren’t cookie-cutter solutions, and they’re not for everyone. But if you’ve been through miscarriage or repeated failed implantation, I’ve seen these approaches be absolute game-changers.
Honestly, I believe immune support is going to become a much bigger part of IVF in the future but you don’t have to wait for the future to start asking these questions now.
