Recently, Monash IVF has faced intense scrutiny after two devastating embryo mix-up incidents—one in Brisbane (2023) and another in Melbourne this year. Both were attributed to “human error” during non-standard procedures. While an independent review confirmed this, the full findings remain private to protect patient confidentiality, a decision that’s sparked widespread criticism. Patients, advocacy groups, and the fertility community are calling for greater transparency, mandatory reporting of adverse events, and consistent national regulation. And honestly, it’s no surprise. Trust in the fertility space is sacred, and when it’s broken, the ripple effects are deeply personal.

My Take...

For women and couples trying to conceive, every embryo represents so much more than biology. It’s years of heartbreak, effort, and hope. When mistakes happen, they deserve honesty, accountability, and reassurance that safeguards are in place to protect what matters most. At the same time, we’re on the brink of a shift. AI and advanced lab automation are already being trialed for embryo selection, quality control, and double-witnessing to reduce human error. While nothing will ever replace the care and compassion of human oversight, I believe AI will play a powerful role in creating safer, more reliable IVF processes and it’s advancing faster than most people realise.For now, the challenge is balance: transparency and accountability today, while embracing the right technology to protect the future.
